Dear Parents and Student-Athletes,
The start of the 2017-2018 Competitive Sports season is rapidly approaching and the Physical Education Department is very excited to get things going! We encourage all Grade Five – Grade Eight families to explore the opportunities of participating in our After-School Sports Program.
Below you will find the link to the Competitive Sports Guidebook for you to read, which will layout the following: sports, objectives for the program, and the expectations for students and parents. It is very important that all parents and student-athletes thoroughly read the guidebook. Once you have read the guidebook, page 9 (Parent Student-Athlete Contract) must be filled out and returned to the Physical Education department no later than September 26, 2017 (click here). If the student-athlete is interested in more than one sport during the school year, please indicate this on the attached contract. Our goal is to make this a one-time process to cover the entire school year.
We also need each student to send in a completed Release of Liability Waiver. This must be completed in full and returned with the Student Athlete Contract by September 26, 2017. This form must be on file prior to engaging in any extra-curricular sport.
Lastly, parents of student-athletes often assist team coaches in driving teammates to away games at other schools in the area. If you are willing and able to help drive during this school year, it is necessary that you complete a Driver Clearance Form. It takes about one week for the School to receive clearance from the DMV once the form is submitted, so it is important to turn in this form as soon as possible. Please note that this driver clearance must be completed every year; if you were cleared last year, you must still complete a new form for this year.
